Here are a bunch of USB fixes for 3.13-rc3.
Nothing major, but we seem to have an argument about a XHCI fix, so I'm not
including a revert that Sarah requested, because that breaks a USB network
driver, and I can't revert the USB network driver fix without reintroducing
other bugs that it fixed. So as it is, everything should now be
working. Worse case, I can revert the XHCI fix before 3.13-final is
out, but it seems to work well here with my testing, so all should be
good.
Other than that, some driver updates based on reports.

Some of KERN_EMERG printk messages do not really deserve this log
level and the one in log_wait_commit() is even rather useless (the
journal has been previously aborted and *that* is where we should have
been complaining). So make some messages just KERN_ERR and remove the
useless message.

If a handle runs out of space, we currently stop the kernel with a BUG
in jbd2_journal_dirty_metadata(). This makes it hard to figure out
what might be going on. So return an error of ENOSPC, so we can let
the file system layer figure out what is going on, to make it more
likely we can get useful debugging information). This should make it
easier to debug problems such as the one which was reported by:

The only two callers of this function are ext4_handle_dirty_metadata()
and ocfs2_journal_dirty(). The ocfs2 function will trigger a
BUG_ON(), which means there will be no change in behavior. The ext4
function will call ext4_error_inode() which will print the useful
debugging information and then handle the situation using ext4's error
handling mechanisms (i.e., which might mean halting the kernel or
remounting the file system read-only).
Also, since both file systems already call WARN_ON(), drop the WARN_ON
from jbd2_journal_dirty_metadata() to avoid two stack traces from
being displayed.

When the filesystem doesn't support extents (like in ext2/3
compatibility modes), there is no need to reserve any clusters. Space
estimates for writing are exact, hole punching doesn't need new
metadata, and there are no unwritten extents to convert.
This fixes a problem when filesystem still having some free space when
accessed with a native ext2/3 driver suddently reports ENOSPC when
accessed with ext4 driver.

That thing should be del_timer_sync(); consider what happens
if ext4_put_super() call of del_timer() happens to come just as it's
getting run on another CPU. Since that timer reschedules itself
to run next day, you are pretty much guaranteed that you'll end up
with kfree'd scheduled timer, with usual fun consequences. AFAICS,
that's -stable fodder all way back to 2010... [the second del_timer_sync()
is almost certainly not needed, but it doesn't hurt either]

027a485d12 ("sysfs: use a separate locking class for open files
depending on mmap") assigned different lockdep key to
sysfs_open_file->mutex depending on whether the file implements mmap
or not in an attempt to avoid spurious lockdep warning caused by
merging of regular and bin file paths.
While this restored some of the original behavior of using different
locks (at least lockdep is concerned) for the different clases of
files. The restoration wasn't full because now the lockdep key
assignment depends on whether the file has mmap or not instead of
whether it's a regular file or not.
This means that bin files which don't implement mmap will get assigned
the same lockdep class as regular files. This is problematic because
file_operations for bin files still implements the mmap file operation
and checking whether the sysfs file actually implements mmap happens
in the file operation after grabbing @sysfs_open_file->mutex. We
still end up adding locking dependency from mmap locking to
sysfs_open_file->mutex to the regular file mutex which triggers
spurious circular locking warning.
Fix it by restoring the original behavior fully by differentiating
lockdep key by whether the file is regular or bin, instead of the
existence of mmap.

This patch allows you to atomically remove all rules stored in
a table via the NFT_MSG_DELRULE command. You only need to indicate
the specific table and no chain to flush all rules stored in that
table.

Add a flag to tell the PCI subsystem that kernel is shutting down in
preparation to kexec a kernel. Add code in PCI subsystem to use this flag
to clear Bus Master bit on PCI devices only in case of kexec reboot.
This fixes a power-off problem on Acer Aspire V5-573G and likely other
machines and avoids any other issues caused by clearing Bus Master bit on
PCI devices in normal shutdown path. The problem was introduced by
b566a22c23 ("PCI: disable Bus Master on PCI device shutdown").

There is an as-yet unexplained bug that sometimes prevents (or delays)
the driver seeing the completion event for a completed MCDI request on
the SFC9120. The requested configuration change will have happened
but the driver assumes it to have failed, and this can result in
further failures. We can mitigate this by polling for completion
after unsuccessfully waiting for an event.

rx_prefix_size is 4-bytes aligned on Falcon/Siena (16 bytes), but it is equal
to 14 on EF10. So, it should be taken into account if arch requires IP header
to be 4-bytes aligned (via NET_IP_ALIGN).

There is a single MCDI PTP operation for setting the frequency
adjustment and applying a time offset to the hardware clock. When
applying a time offset we should not change the frequency adjustment.
These two operations can now be requested separately but this requires
a flash firmware update. Keep using the single operation, but
remember and repeat the previous frequency adjustment.

This disables PTP when we bring the interface down to avoid getting
unmatched RX timestamp events, and tries to re-enable it when bringing
the interface up.

In case of a flood of PTP packets, the timestamp peripheral and MC
firmware on the SFN[56]322F boards may not be able to provide
timestamp events for all packets. Don't complain too much about this.

The am3517 is wrongly booting as omap3 which means that the am3517
specific devices like Ethernet won't work when booted with device
tree. Now with the new devices defined in am3517.dtsi, let's use
that instead of the omap3.dtsi, and add a separate machine entry
for am3517 so am3517-evm can use it.

That's not correct way of doing things in this case because these are
separate devices with their own address space, interrupts, SYSCONFIG
registers and can set their PM states independently.
So they should all be fixed up to be separate devices in the .dts files.
We also have the related data removed for at least omap4 in commit
3b9b10151c (ARM: OMAP4: hwmod data: Clean up the data file), so
that data is wrongly initialized as null data.
So we need to fix two bugs:
1. We are only checking the first entry of the ti,hwmods property
This means that we're only initializing the first hwmods entry
instead of the ones listed in the ti,hwmods property.
2. We are only checking the child nodes, not the nodes themselves
This means that anything listed at OCP level is currently just
ignored and unitialized and at least the omap4 case, with the
legacy data missing from the hwmod.
Fix both of the issues by using an index to the ti,hwmods property
and changing the hwmod lookup function to also check the current node
for ti,hwmods property instead of just the children.
While at it, let's also add some warnings for the bad data so it's
easier to fix.

The current code unmaps the DMA mapping created for rx skb_buff's by
using the data_size as the the mapping size. This is wrong since the
correct size to specify should match the size used to create the mapping.

br_stp_rcv() is reached by non-rx_handler path. That means there is no
guarantee that dev is bridge port and therefore simple NULL check of
->rx_handler_data is not enough. There is need to check if dev is really
bridge port and since only rcu read lock is held here, do it by checking
->rx_handler pointer.
Note that synchronize_net() in netdev_rx_handler_unregister() ensures
this approach as valid.

efx_ptp_is_ptp_tx() must be robust against skbs from raw sockets that
have invalid IPv4 and UDP headers.
Add checks that:
- the transport header has been found
- there is enough space between network and transport header offset
for an IPv4 header
- there is enough space after the transport header offset for a
UDP header

There's an issue when showing the value of packets_per_slave due to
using signed integer. The value may be < 0 and thus not put through
reciprocal_value() before showing. This patch makes it use unsigned
integer when showing it.

ae7f164a09 ("cgroup: move cgroup->subsys[] assignment to
online_css()") moved cgroup->subsys[] assignements later in
cgroup_create() but didn't update error handling path accordingly
leading to the following oops and leaking later css's after an
online_css() failure. The oops is from cgroup destruction path being
invoked on the partially constructed cgroup which is not ready to
handle empty slots in cgrp->subsys[] array.

This patch moves reference bumping inside online_css() loop, clears
css_ar[] as css's are brought online successfully, and updates
err_destroy path so that either a css is fully online and destroyed by
cgroup_destroy_locked() or the error path frees it. This creates a
duplicate css free logic in the error path but it will be cleaned up
soon.
v2: Li pointed out that cgroup_destroy_locked() would do NULL-deref if
invoked with a cgroup which doesn't have all css's populated.
Update cgroup_destroy_locked() so that it skips NULL css's.

During suspend-resume and lancer error recovery we will cleanup and
re-initialize the resources through be_clear() and be_setup() respectively.
During re-initialisation in be_setup(), if be_get_config() fails, we'll again
call be_clear() which will cause a NULL pointer dereference as adapter->pmac_id is
already freed.

The Firmware update would be detected by looking at the sliport_error1/
sliport_error2 register values(0x02/0x00). If its not a FW reset the current
messaging would take place. If the error is due to FW reset, log a message to
user that "Firmware update in progress" and also do not log sliport_status and
sliport_error register values.

This failure represents a hypervisor issue, but if it does occur then nothing
good can come of returning pages which still refer to a foreign owned page
into the general allocation pool.
Instead we are forced to leak them. Log that we have done so.
The potential for failure only exists for autotranslated guest (e.g. ARM and
x86 PVH).

Write-combine and cacheable mappings use Normal memory on arm64. On SMP
systems, the pte needs the shareability bit which is set in
pgprot_default. Use this for defining PROT_DEFAULT used by ioremap_wc
and ioremap_cache (Device memory is shareable by default, does not need
additional attributes).

The refactoring of el2_setup split code setting up EL2 and detecting the
CPU boot mode in separate chunks. This allows the code that sets up EL2 to
run in an endian independent way - ie before the endianess is set up in
the respective sctlr registers.
This patch brings secondary_entry up-to-date so that CPUs entering the
kernel through this code path set-up EL2 and the cpu boot mode properly.

Currently there is no dsb between the tlbi in __cpu_setup and the write
to SCTLR_EL1 which enables the MMU in __turn_mmu_on. This means that the
TLB invalidation is not guaranteed to have completed at the point
address translation is enabled, leading to a number of possible issues
including incorrect translations and TLB conflict faults.
This patch moves the tlbi in __cpu_setup above an existing dsb used to
synchronise I-cache invalidation, ensuring that the TLBs have been
invalidated at the point the MMU is enabled.

When handling mapping requests, we dereference the SMMU domain before
checking that it is NULL. This patch fixes the issue by removing the check
altogether, since we don't actually use the leaf_smmu when creating
mappings.

When creating IO mappings, we lazily allocate our page tables using the
standard, non-atomic allocator functions. This presents us with a
problem, since our page tables are protected with a spinlock.
This patch reworks the smmu_domain lock to use a mutex instead of a
spinlock. iova_to_phys is then reworked so that it only reads the page
tables, and can run in a lockless fashion, leaving the mutex to guard
against concurrent mapping threads.
