d1bbd20972 firewire: allow explicit flushing of iso packet completions
Extend the kernel and userspace APIs to allow reporting all currently
completed isochronous packets, even if the next interrupt packet has not
yet been reached.  This is required to determine the status of the
packets at the end of a paused or stopped stream, and useful for more
precise synchronization of audio streams.

18d627113b firewire: prevent dropping of completed iso packet header data
The buffer for the header data of completed iso packets has a fixed
size, so it is possible to configure a stream with a big interval
between interrupt packets or with big headers so that this buffer would
overflow.  Previously, ohci.c would drop any data that would not fit,
but this could make unsuspecting applications believe that fewer than
the actual number of packets have completed.

Instead of dropping data, add calls to flush_iso_completion() so that
there are as many events as needed to report all of the data.

910e76c607 firewire: ohci: factor out iso completion flushing code
In preparation for the following patches that add more flushing, move
the code for flushing accumulated header data into a common function.
The timestamp of the last completed packed is passed through the context
structure instead of a function parameter to allow accessing this value
later outside of the handle_i?_packet functions.

73864012f3 firewire: ohci: simplify iso header pointer arithmetic
When storing the header data of completed iso packets, we effectively
treat the buffers as arrays of quadlets.  Actually declaring the
pointers as u32* avoids repetitive pointer arithmetic, removes the
unhelpfully named "i" variables, and thus makes the code clearer.

The comment incorrectly talked about one little-endian quadlet, while
there are actually two.  Furthermore, the endianness of the remaining
headers depends on whatever protocol is used, so don't mention them.

5407b14c67 gianfar: use netif_tx_queue_stopped instead of __netif_subqueue_stopped
The __netif_subqueue_stopped() just does the following:

        struct netdev_queue *txq = netdev_get_tx_queue(dev, queue_index);
        return netif_tx_queue_stopped(txq);

and since we already have the txq in scope, we can just call that
directly in this case.

93dc6107a7 Don't limit non-nested epoll paths
Commit 28d82dc1c4 ("epoll: limit paths") that I did to limit the
number of possible wakeup paths in epoll is causing a few applications
to longer work (dovecot for one).

The original patch is really about limiting the amount of epoll nesting
(since epoll fds can be attached to other fds). Thus, we probably can
allow an unlimited number of paths of depth 1. My current patch limits
it at 1000. And enforce the limits on paths that have a greater depth.

187e70a554 ib_srpt: Fix srpt_handle_cmd send_ioctx->ioctx_kref leak on exception
This patch addresses a bug in srpt_handle_cmd() failure handling where
send_ioctx->kref is being leaked with the local extra reference after init,
causing the expected kref_put() in srpt_handle_send_comp() to not be the final
call to invoke srpt_put_send_ioctx_kref() -> transport_generic_free_cmd() and
perform se_cmd descriptor memory release.

It also fixes a SCF_SCSI_RESERVATION_CONFLICT handling bug where this code
is incorrectly falling through to transport_handle_cdb_direct() after
invoking srpt_queue_status() to send SAM_STAT_RESERVATION_CONFLICT status.

Note this patch is for >= v3.3 mainline code, and current lio-core.git
code has already been converted to target_submit_cmd() + se_cmd->cmd_kref usage,
and internal ioctx->kref usage has been removed.  I'm including this patch
now into target-pending/for-next with a CC' for v3.3 stable.

ee9b866a36 loopback: Fix transport_generic_allocate_tasks error handling
This patch addresses a tcm_loop bug with transport_generic_allocate_tasks()
return checking in tcm_loop_submission_work() where other non zero return
codes (including -EBUSY for reservation conflicts) are incorrectly falling
through to transport_generic_map_mem_to_cmd() -> transport_handle_cdb_direct().

This bug was introduced into target-pending/for-next-merge with the following
for-3.4 commit:

70ac07bb63 ASoC: fsl: p1022ds: tell the WM8776 codec driver that it's the master
The WM8776 codec driver requires the machine driver to set one of the
SND_SOC_DAIFMT_CBx_xxx values.  The P1022DS machine driver should be setting
SND_SOC_DAIFMT_CBM_CFM, but since that value was zero, no one noticed.

Commit 75d9ac46 ("ASoC: Allow DAI formats to be specified in the
dai_link"), however, changed the value of SND_SOC_DAIFMT_CBM_CFM from zero
to a non-zero value, which means that it now needs to be specifically set
by the machine driver.

We also set SND_SOC_DAIFMT_NB_NF, for the same reason.

ab5f299f51 PM / devfreq: add relation of recommended frequency.
The semantics of "target frequency" given to devfreq driver from
devfreq framework has always been interpretted as "at least" or GLB
(greatest lower bound). However, the framework might want the
device driver to limit its max frequency (LUB: least upper bound),
especially if it is given by thermal framework (it's too hot).

Thus, the target fuction should have another parameter to express
whether the framework wants GLB or LUB. And, the additional parameter,
"u32 flags", does it.

With the update, devfreq_recommended_opp() is also updated.

e49a29bd0e Try using machine credentials for RENEW calls
Using user credentials for RENEW calls will fail when the user
credentials have expired.

To avoid this, try using the machine credentials when making RENEW
calls. If no machine credentials have been set, fall back to using user
credentials as before.

9390f42546 NFSv4.1: Fix a few issues in filelayout_commit_pagelist
- Fix a race in which NFS_I(inode)->commits_outstanding could potentially
  go to zero (triggering a call to nfs_commit_clear_lock()) before we're
  done sending out all the commit RPC calls.

- If nfs_commitdata_alloc fails, there is no reason why we shouldn't
  try to send off all the commits-to-ds.

- Simplify the error handling.

- Change pnfs_commit_list() to always return either
  PNFS_ATTEMPTED or PNFS_NOT_ATTEMPTED.

8dd3775889 NFSv4.1: Clean ups and bugfixes for the pNFS read/writeback/commit code
Move more pnfs-isms out of the generic commit code.

Bugfixes:

- filelayout_scan_commit_lists doesn't need to get/put the lseg.
  In fact since it is run under the inode->i_lock, the lseg_put()
  can deadlock.

- Ensure that we distinguish between what needs to be done for
  commit-to-data server and what needs to be done for commit-to-MDS
  using the new flag PG_COMMIT_TO_DS. Otherwise we may end up calling
  put_lseg() on a bucket for a struct nfs_page that got written
  through the MDS.

- Fix a case where we were using list_del() on an nfs_page->wb_list
  instead of list_del_init().

- filelayout_initiate_commit needs to call filelayout_commit_release
  on error instead of the mds_ops->rpc_release(). Otherwise it won't
  clear the commit lock.

Cleanups:

- Let the files layout manage the commit lists for the pNFS case.
  Don't expose stuff like pnfs_choose_commit_list, and the fact
  that the commit buckets hold references to the layout segment
  in common code.

- Cast out the put_lseg() calls for the struct nfs_read/write_data->lseg
  into the pNFS layer from whence they came.

- Let the pNFS layer manage the NFS_INO_PNFS_COMMIT bit.

a7611e84bd firewire: cdev: fix IR multichannel event documentation
State more precisely when fw_cdev_event_iso_interrupt_mc is sent.
While the comment tried to reflect an amibuity in the OHCI
specification, there is only one method that is useful in practice,
and this also matches all the hardware implementations I've tested.

0c0efbacab firewire: ohci: fix too-early completion of IR multichannel buffers
handle_ir_buffer_fill() assumed that a completed descriptor would be
indicated by a non-zero transfer_status (as in most other descriptors).
However, this field is written by the controller as soon as (the end of)
the first packet has been written into the buffer.  As a consequence, if
we happen to run into such a descriptor when the interrupt handler is
executed after such a packet has completed, the descriptor would be
taken out of the list of active descriptors as soon as the buffer had
been partially filled, so the event for the buffer being completely
filled would never be sent.

To fix this, handle descriptors only when they have been completely
filled, i.e., when res_count == 0.  (This also matches the condition
that is reported by the controller with an interrupt.)

a16a1647fa netfilter: ctnetlink: fix race between delete and timeout expiration
Kerin Millar reported hardlockups while running `conntrackd -c'
in a busy firewall. That system (with several processors) was
acting as backup in a primary-backup setup.

After several tries, I found a race condition between the deletion
operation of ctnetlink and timeout expiration. This patch fixes
this problem.
