8b9d966665 dm cache: fix truncation bug when copying a block to/from >2TB fast device
During demotion or promotion to a cache's >2TB fast device we must not
truncate the cache block's associated sector to 32bits.  The 32bit
temporary result of from_cblock() caused a 32bit multiplication when
calculating the sector of the fast device in issue_copy_real().

Use an intermediate 64bit type to store the 32bit from_cblock() to allow
for proper 64bit multiplication.

Here is an example of how this bug manifests on an ext4 filesystem:

 EXT4-fs error (device dm-0): ext4_mb_generate_buddy:756: group 17136, 32768 clusters in bitmap, 30688 in gd; block bitmap corrupt.
 JBD2: Spotted dirty metadata buffer (dev = dm-0, blocknr = 0). There's a risk of filesystem corruption in case of system crash.

596f3142d2 KVM: SVM: fix cr8 intercept window
We always disable cr8 intercept in its handler, but only re-enable it
if handling KVM_REQ_EVENT, so there can be a window where we do not
intercept cr8 writes, which allows an interrupt to disrupt a higher
priority task.

Fix this by disabling intercepts in the same function that re-enables
them when needed. This fixes BSOD in Windows 2008.

ac93ac7403 PCI: Don't check resource_size() in pci_bus_alloc_resource()
Paul reported that after f75b99d5a7 ("PCI: Enforce bus address limits in
resource allocation") on a 32-bit kernel (CONFIG_PHYS_ADDR_T_64BIT not
set), intel-gtt complained "can't ioremap flush page - no chipset
flushing".  In addition, other PCI resource allocations, e.g., for bridge
windows, failed.

This happens because we incorrectly skip bus resources of
[mem 0x00000000-0xffffffff] because we think they are of size zero.
When resource_size_t is 32 bits wide, resource_size() on
[mem 0x00000000-0xffffffff] returns 0 because (r->end - r->start + 1)
overflows.

Therefore, we can't use "resource_size() == 0" to decide that allocation
from this resource will fail.  allocate_resource() should fail anyway if it
can't satisfy the address constraints, so we should just depend on that.

A [mem 0x00000000-0xffffffff] bus resource is obviously not really valid,
but we do fall back to it as a default when we don't have information about
host bridge apertures.

3cdeb713dc PCI: Enable INTx in pci_reenable_device() only when MSI/MSI-X not enabled
Andreas reported that after 1f42db786b ("PCI: Enable INTx if BIOS left
them disabled"), pciehp surprise removal stopped working.

This happens because pci_reenable_device() on the hotplug bridge (used in
the pciehp_configure_device() path) clears the Interrupt Disable bit, which
apparently breaks the bridge's MSI hotplug event reporting.

Previously we cleared the Interrupt Disable bit in do_pci_enable_device(),
which is used by both pci_enable_device() and pci_reenable_device().  But
we use pci_reenable_device() after the driver may have enabled MSI or
MSI-X, and we *set* Interrupt Disable as part of enabling MSI/MSI-X.

This patch clears Interrupt Disable only when MSI/MSI-X has not been
enabled.

5c673b60a9 drm/i915: Don't enable display error interrupts from the start
We need to enable interrupt processing before all the modeset
state is set up. But that means we can fall over when we get a pipe
underrun. This shouldn't happen as long as the bios works correctly
but as usual this turns out to be wishful thinking.

So disable error interrupts at irq install time and rely on the
re-enabling code in the modeset functions to take care of this.

Note that due to the SDE interrupt handling race we must
uncondtionally enable all interrupt sources in SDEIER, hence no need
to enable the SERR bit specifically.

On gmch platforms we don't have an explicit enable/mask bit for fifo
underruns. Fixing this up would require a bit of software tracking,
hence is material for a separate patch. To make this possible we need
to switch all gmch platforms to the new pipestat interrupt handling
scheme Imre implemented for vlv, and then also add a safe form of sw
state checking to __cpu_fifo_underrun_reporting_enabled a bit.

v2: Also handle the ilk/snb cpu fifo underrun bits accordingly.

2430262493 drm/i915: Fix scanline counter fixup on BDW
The display interrupts changed on BDW, so the current ILK-HSW specific
code in ilk_pipe_in_vblank_locked() doesn't work there. Add the required
bits for BDW, and while at it, change the existing code to use nicer
looking vblank status bit macros.

Also remove the now stale __raw_i915_read16() definition which was
left over from the failed gen2 ISR experiment.

fcb818231f drm/i915: Add a workaround for HSW scanline counter weirdness
On HSW the scanline counter seems to behave differently depending on
the output type. eDP on port A does what you would expect an the normal
+1 fixup is sufficient to cover it. But on HDMI outputs we seem to need
a +2 fixup. Just assume we always need the +2 fixup and accept the
slight inaccuracy on eDP.

This fixes a regression introduced in:

    drm/radeon: Move the early vblank IRQ fixup to radeon_get_crtc_scanoutpos()

That commit removed the heuristic that tried to fix up the timestamps
for vblank interrupts that fire a bit too early. Since then the vblank
timestamp code would treat some vblank interrupts as spurious since the
scanline counter would indicate that vblank_start wasn't reached yet.
That in turn lead to incorrect vblank event sequence numbers being
reported to userspace, which lead to unsteady framerate in applications
such as XBMC which uses them for timing purposes.

c1bacbae81 genirq: Provide irq_request/release_resources chip callbacks
For certain irq types, e.g. gpios, it's necessary to request resources
before starting up the irq.

This might fail so we cannot use the irq_startup() callback because we
might call the irq_set_type() callback before that which does not make
sense when the resource is not available. Calling irq_startup() before
irq_set_type() can lead to spurious interrupts which is not desired
either.

This patch fixes an error check while using of_irq_to_resource.
of_irq_to_resource returns non-zero interrupt number on success and zero
on error. The driver was using error check is wrong way.

Without this patch the driver will configure interrupt zero if there is
no interrupt specified in the node.

These driver files were also auto-generated, which should allow us to
make e.g. the U-Boot drivers completely consistent with the kernel in
the future:-)

ce43625466 pinctrl: tegra: dynamically calculate function list of groups
The per-SoC data structures for Tegra pinctrl stored some information
in a redundant way. Specifically, the list of groups that each function
could be muxed onto was stored once explicitly, and also as part of the
definition of each group. Eliminate this redundancy, and calculate each
function's list of valid groups at pinctrl probe time. This removes
thousands of lines of code from the pinctrl driver and ~16K from the
vmlinux binary size, and adds only about 500uS to the boot process (on
Tegra30; newer SoCs will likely be faster still).

4e6a609fb7 pinctrl: st: Enhance the controller to manage unavailable registers
This patch adds a new logic inside the st pinctrl to manage
an unsupported scenario: some sysconfig are not available!

This is the case of STiH407 where, although documented, the
following registers from SYSCFG_FLASH have been removed from the SoC.

SYSTEM_CONFIG3040
   Output Enable pad control for all PIO Alternate Functions
and
SYSTEM_ CONFIG3050
   Pull Up pad control for all PIO Alternate Functions

Without managing this condition an imprecise external abort
will be detect.

To do this the patch also reviews the st_parse_syscfgs
and other routines to manipulate the registers only if
actually available.
In any case, for example the st_parse_syscfgs detected
an error condition but no action was made in the
st_pctl_probe_dt.

6aced33f49 pinctrl: msm: drop wake_irqs bitmap
Currently, the wake_irqs bitmap is used to track whether there are any
gpio's which are configured as wake irqs, and uses this to determine
whether or not to call enable_irq_wake()/disable_irq_wake() on the
summary interrupt.

However, the genirq core already handles this case, by maintaining a
'wake_count' per irq_desc, and only calling into the controlling
irq_chip when wake_count transitions 0 <-> 1.

Drop this bitmap, and unconditionally call irq_set_irq_wake() on the
summary interrupt.

0e6d6ec02f drm/ttm: Work around performance regression with VM_PFNMAP
A performance regression was introduced in TTM in linux 3.13 when we started using
VM_PFNMAP for shared mappings. In theory this should've been faster due to
less page book-keeping but it appears like VM_PFNMAP + x86 PAT + write-combine
is a particularly cpu-hungry combination, as seen by largely increased
cpu-usage on r200 GL video playback.

Until we've sorted out why, revert to always use VM_MIXEDMAP.

6f008e72cd locking/mutex: Fix debug checks
OK, so commit:

  1d8fe7dc80 ("locking/mutexes: Unlock the mutex without the wait_lock")

generates this boot warning when CONFIG_DEBUG_MUTEXES=y:

And that makes sense, because as soon as we release the lock a
new owner can come in...

One would think that !__mutex_slowpath_needs_to_unlock()
implementations suffer the same, but for DEBUG we fall back to
mutex-null.h which has an unconditional 1 for that.

The mutex debug code requires the mutex to be unlocked after
doing the debug checks, otherwise it can find inconsistent
state.

1ed71e59bc parisc: Validate online cpus in irq_set_affinity() callbacks
The [user space] interface does not filter out offline cpus. It merily
guarantees that the mask contains at least one online cpu.

So the selector in the irq chip implementation needs to make sure to
pick only an online cpu because otherwise:

     Offline Core 1
     Set affinity to 0xe (is valid due to online mask 0xd)
     cpumask_first will pick core 1, which is offline

The user space interface does not filter out offline cpus. It merily
verifies that the mask contains at least one online cpu. So the
selector in the irq chip implementation needs to make sure to pick
only an online cpu because otherwise:

     Offline Core 1
     Set affinity to 0xe
     Selector will pick first set bit, i.e. core 1

383afd0971 sched: Fix broken setscheduler()
I decided to run my tests on linux-next, and my wakeup_rt tracer was
broken. After running a bisect, I found that the problem commit was:

   linux-next commit c365c292d0
   "sched: Consider pi boosting in setscheduler()"

And the reason the wake_rt tracer test was failing, was because it had
no RT task to trace. I first noticed this when running with
sched_switch event and saw that my RT task still had normal SCHED_OTHER
priority. Looking at the problem commit, I found:

 -       p->normal_prio = normal_prio(p);
 -       p->prio = rt_mutex_getprio(p);

With no

 +       p->normal_prio = normal_prio(p);
 +       p->prio = rt_mutex_getprio(p);

Reading what the commit is suppose to do, I realize that the p->prio
can't be set if the task is boosted with a higher prio, but the
p->normal_prio still needs to be set regardless, otherwise, when the
task is deboosted, it wont get the new priority.

The p->prio has to be set before "check_class_changed()" is called,
otherwise the class wont be changed.

Also added fix to newprio to include a check for deadline policy that
was missing. This change was suggested by Juri Lelli.

Commit a998d43423 claimed to introduce negative offset support to x86 jit,
but it couldn't be working, since at the time of the execution
of LD+ABS or LD+IND instructions via call into
bpf_internal_load_pointer_neg_helper() the %edx (3rd argument of this func)
had junk value instead of access size in bytes (1 or 2 or 4).

Store size into %edx instead of %ecx (what original commit intended to do)

20a599bec9 bridge: multicast: enable snooping on general queries only
Without this check someone could easily create a denial of service
by injecting multicast-specific queries to enable the bridge
snooping part if no real querier issuing periodic general queries
is present on the link which would result in the bridge wrongly
shutting down ports for multicast traffic as the bridge did not learn
about these listeners.

With this patch the snooping code is enabled upon receiving valid,
general queries only.

9ed973cc40 bridge: multicast: add sanity check for general query destination
General IGMP and MLD queries are supposed to have the multicast
link-local all-nodes address as their destination according to RFC2236
section 9, RFC3376 section 4.1.12/9.1, RFC2710 section 8 and RFC3810
section 5.1.15.

Without this check, such malformed IGMP/MLD queries can result in a
denial of service: The queries are ignored by most IGMP/MLD listeners
therefore they will not respond with an IGMP/MLD report. However,
without this patch these malformed MLD queries would enable the
snooping part in the bridge code, potentially shutting down the
according ports towards these hosts for multicast traffic as the
bridge did not learn about these listeners.

2449d33a40 cpufreq: SPEAr: Instantiate as platform_driver
As multiplatform build is being adopted by more and more
ARM platforms, initcall function should be used very carefully.
For example, when SPEAr cpufreq driver is enabled on a kernel
booted on a non-SPEAr board, we will get following boot time error:

	spear_cpufreq: Invalid cpufreq_tbl

To eliminate this undesired the effect, the patch changes SPEAr
driver to have it instantiated as a platform_driver. Then it will
only run on platforms that create the platform_device "spear-cpufreq".

This patch also creates platform node for SPEAr13xx boards.

distinguish between CPU offline/online on suspend/resume vs sysfs

e0b3165ba5 cpufreq: add 'freq_table' in struct cpufreq_policy
freq table is not per CPU but per policy, so it makes more sense to
keep it within struct cpufreq_policy instead of a per-cpu variable.

This patch does it. Over that, there is no need to set policy->freq_table
to NULL in ->exit(), as policy structure is going to be freed soon.
