There are two modprobe parameters for VBI that aren't used
anywhere (one for debug, the other one related to the buffer
size). Get rid of them!

The em28xx scaler data are used only for analog video. Move them to
struct em28xx_v4l2.

This patch converts the em28xx driver over to videobuf2. It is
likely that em28xx_fh can go away entirely, but that will come in
a separate patch.

Also rename the corresponding field isoc_ctl in struct em28xx
to usb_ctl.
We will use this struct for USB bulk transfers, too.
