commit 859b7b649a introduced the ability to call
fcoe_recv_frame in softirq context. While this is beneficial to performance,
its not safe to do, as it breaks the serialization of access to the lport
structure (i.e. when an fcoe interface is being torn down, theres no way to
serialize the teardown effort with the completion of receieve operations
occuring in softirq context. As a result, lport (and other) data structures can
be read and modified in parallel leading to corruption. Most notable is the
vport list, which is protected by a mutex, that will cause a panic if a softirq
receive while said mutex is locked. Additionaly, the ema_list, discussed here:

Can be corrupted if a list traversal occurs in softirq context at the same time
as a list delete in process context. And generally the lport state variables
will not be stable, and may lead to unpredictable results.
The most direct fix is to remove the bits from the above commit that allowed
fcoe_recv_frame to be called in softirq context. We just force all frames to be
handled by the per-cpu rx threads. This will allow the fcoe_if_destroy's use of
fcoe_percpu_clean to function properly, ensuring that no frames are being
received while the lport is being torn down.

Xen dom0 needs to paravirtualize IO operations to the IO APIC,
so add a io_apic_ops for it to intercept. Do this as ops
structure because there's at least some chance that another
paravirtualized environment may want to intercept these.

Polling for card hotplug events is a better option than using SDHI native
hotplug interrupt, because the latter option forces the controller to stay
permanently powered up and enabled.

Now that userspace is making use of kernel-provided sanitized headers for
working out supported interfaces, we need to be a bit more diligent with
matching the syscall definitions with their actual wiring/support state.
In theory it shouldn't hurt anything since sys_ni_syscall will ultimately
do the right thing, but there's also not much need to lie about legacy
x86 syscalls that we've never supported.
This tightens things up a bit for uClibc at least.

Dan Carpenter noticed that ixgbevf initial default was different than
the rest. But the problem is broader than that, only one Intel driver (ixgb)
was doing it almost right.
The convention for default debug level should be consistent among
Intel drivers and follow established convention.

This A to B sequence is not reentrant, since controlling variables
(for example, cookie_tx above) are not queues nor lists. So, they
must be invoked as A B A B..., otherwise results in kernel crash.
To ensure the sequence, sci_start_tx() seems to test if cookie_tx < 0
(represents "not used") to call schedule_work().
But cookie_tx will not be set (to a cookie, also means "used") until
in the middle of work queue scheduled function work_fn_tx().
This gap between the test and set allows the breakage of the sequence
under the very frequently call of uart_start().
Another gap between async_tx_ack() and another schedule_work() results
in the same issue, too.
This patch introduces a new condition "cookie_tx == 0" just to mark
it is "busy" and assign it within spin-locked region to fill the gaps.

Merge branch 'fixes-non-critical' into next/drivers2
fixes-non-critical has already been merged upstream, and there are a
handful of annoying context conflicts that it makes sense to resolve
before sending up. Part of this is because there was parallel development
going on between more urgent fixes and general driver fixups, it should

There are two channels here. User space starts counting channels at one
but in the kernel we start at zero. If the user passes in a zero

The flow in which the bnx2x driver starts after a previous driver
has been terminated in an 'unclean' manner has several bugs and
FW risks, which makes it possible for the driver to fail after
boot-from-SAN or kdump.
This patch contains a revised flow which performs a safer
initialization, solving the possible crash scenarios.
Notice this patch contains lines with over 80 characters, as it
keeps print-strings in a single line.

This happened on a machine with a custom hotplug script calling nameif,
probably due to slow firmware loading. At the time nameif uses ethtool
to gather interface information, i2400m->fw_name is zero and so a null
pointer dereference occurs from within i2400m_get_drvinfo().

so the current tests for 'NFS4ERR_DELEG_REVOKED' etc. are all being
ignored.
Secondly, clean up the code so that we only need to test

We can currently loop forever in nfs4_lookup_root() and in
nfs41_proc_secinfo_no_name(), if the first iteration returns a
NFS4ERR_DELAY or something else that causes exception.retry to get

This patch fixes a regression introduced by commit "e1000: do vlan
cleanup (799d531)".
Apparently some e1000 chips (not mine) are sensitive about the order of
setting vlan filter and vlan stripping/inserting functionality. So this
patch changes the order so it's the same as before vlan cleanup.

970 and Power4 don't support "continuous sampling" which means that
when we aren't in marked instruction sampling mode (marked events),
SIAR isn't updated with the last instruction sampled before the
perf interrupt. On those processors, we must thus use the exception
SRR0 value as the sampled instruction pointer.
Those processors also don't support the SIPR and SIHV bits in MMCRA
which means we need some kind of heuristic to decide if SIAR values
represent kernel or user addresses.

We have code to take environmental and power warning (EPOW)
interrupts but it simply prints a terse error message:
EPOW <0x6240040000000b8 0x0 0x0>
which tells us nothing about what happened. Even worse, if we
don't correctly respond to the interrupt we may get terminated
by firmware.
Add code to printk some useful information when we get EPOW events.
We want to make it clear that we have an error, that it was
reported by firmware and that the RTAS error log will have more
detailed information. eg:
Ambient temperature too high reported by firmware.
Check RTAS error log for details
Depending on the error encountered, we now issue an immediate or
an orderly power down.
Move initialization of the EPOW interrupt earlier in boot since we
want to respond to them as early as possible.

The IO event interrupt code has a function that finds specific
sections in an RTAS error log. We want to use it in the EPOW
code so make it global.
