eb70e5ab8f usb: chipidea: add host role
This adds EHCI host support to the chipidea driver. We want it to be
part of the hdrc driver and not a standalone (sub-)driver module, as
the structure of ehci-hcd.c suggests, so for chipidea controller we
hack it to not provide platform-related code, but only the ehci hcd.

The ehci-platform driver won't work for us here too, because the
controller uses the same registers for both device and host mode and
also otg-related bits, so it's not really possible to put ehci registers
into a separate resource.

This is not a pretty solution, but the alternative is exporting symbols
from the chipidea driver to a ehci-chipidea driver and doing all the
module refcounting.

5f36e231e9 usb: chipidea: add support for roles
Add some generic code for roles and implement simple role switching
based on ID pin state and/or a sysfs file. At this, we also rename
the device to ci_hdrc, which is what it is.

The "manual" switch is made into a sysfs file and not debugfs, because
it might be useful even in non-debug context. For some boards, like
sheevaplug, it seems to be the only way to switch roles without modifying
the hardware, since the ID pin is always grounded.

 "This patch removes some incorrect legacy code to free se_lun_acl
  memory in the NodeACL release path that could potentially trigger an

  That said, we've been able to trigger an OOPS in v4.0 code for this
  special case when the associated MappedLUNs had not also been made
  explicit based on active TPG LUN layout during the conversion, so it
  really makes senses to go ahead and drop this extra cruft to avoid any
  possible issues here.

  This ends up only effecting iscsi-target module code (it's the only
  user) and is CC'ed to stable."

bc25a80d12 usb: move ci13xxx and related code to drivers/usb/chipidea
Since chipidea is a dual role controller, it makes sense to move it
to its own directory, where we can also have host, otg and platform
code related to this controller. It also makes sense to break out
the driver into several compilation units like udc, host, debugging
code, etc.

Firstly, let's move the udc and platform code to drivers/usb/chipidea.

ef15e5490e usb: gadget: ci13xxx: don't use "advance" feature when setting address
Newer versions of the chipidea controller support the "advance" setting
of usb address, which means instead of setting it immediately, deferring
it till the status completion. Unfortunately, older versions of the
controller don't have this feature, so in order to support those too, we
simply don't use it. It's about 4 lines of extra code, and isn't in any
way critical to performance. While at it, change the return value of the
hw_usb_set_address() to void, since it can't fail in any measurable way.

With this patch, ci13xxx_udc driver works with the chipidea controller in
kirkwood (feroceon SoC), as found in, for example, sheevaplug.

7c0482e3d0 powerpc/irq: Fix another case of lazy IRQ state getting out of sync
So we have another case of paca->irq_happened getting out of
sync with the HW irq state. This can happen when a perfmon
interrupt occurs while soft disabled, as it will return to a
soft disabled but hard enabled context while leaving a stale
PACA_IRQ_HARD_DIS flag set.

This patch fixes it, and also adds a test for the condition
of those flags being out of sync in arch_local_irq_restore()
when CONFIG_TRACE_IRQFLAGS is enabled.

This helps catching those gremlins faster (and so far I
can't seem see any anymore, so that's good news).

If a link change interrupt comes in we just clear the interrupt
and continue along without notifying the upper networking layers
that the link has changed. Use the mii_check_link() function to
update the link status whenever a link change interrupt occurs.

6286d85e8e USB: cdc-wdm: remove from device list on disconnect
Prevents dereferencing an invalid struct usb_interface
pointer.

Always delete entry from device list whether or not the
rest of the device state cleanup is postponed. The device
list uses desc->intf as key, and wdm_open will dereference
this key while searching for a matching device.  A device
should not appear in the list unless probe() has succeeded
and disconnect() has not finished.

6b0b79d388 USB: cdc-wdm: cannot use dev_printk when device is gone
We cannot dereference a removed USB interface for
dev_printk. Use pr_debug instead where necessary.

Flush errors are expected if device is unplugged and are
therefore best ingored at this point.

Move the kill_urbs() call in wdm_release with dev_dbg()
for the non disconnect, as we know it has already been
called if WDM_DISCONNECTING is set.  This does not
actually fix anything, but keeps the code more consistent.

cfebf8f42f target: Drop incorrect se_lun_acl release for dynamic -> explict ACL conversion
This patch removes some potentially problematic legacy code within
core_clear_initiator_node_from_tpg() that was originally intended to
release left over se_lun_acl setup during dynamic NodeACL+MappedLUN
generate when running with TPG demo-mode operation.

Since we now only ever expect to allocate and release se_lun_acl from
within target_core_fabric_configfs.c:target_fabric_make_mappedlun() and
target_fabric_drop_mappedlun() context respectively, this code for
demo-mode release is incorrect and needs to be removed.

2a96536e77 iommu/exynos: Add iommu driver for EXYNOS Platforms
This is the System MMU driver and IOMMU API implementation for
EXYNOS SoC platforms. EXYNOS platforms has more than 10 System
MMUs dedicated for each multimedia accelerators.

The System MMU driver is already in arc/arm/plat-s5p but it is
moved to drivers/iommu due to Ohad Ben-Cohen gathered IOMMU
drivers there.

Any device driver in EXYNOS platforms that needs to control its
System MMU must call platform_set_sysmmu() to inform System MMU
driver who will control it. platform_set_sysmmu() is defined in
<mach/sysmmu.h>
